The photograph should meet the following requirements: · Must be taken within the last 3 months · Must be in color · Must be taken against a white background with a matt or semi-matt finish · The photograph image must show the full face and without headgear (headgear worn in accordance with religious or racial customs is acceptable but must not hide the facial features)
· Local sponsor NRIC
· Applicant’s marriage certificate (original and authenticated copy). To provide official translation if document is not in English
· A letter from the Embassy or High Commission of applicant’s country in Singapore stating that the newborn child will follow the parent’s nationality and a valid travel document of that country will be issued to the newborn child.
· Applicant’s travel document with at least 6 months validity.
5. When application for permission to deliver child in Singapore is approved:
· A security deposit of between S$ 1,000 to S$ 3,000 may be required from patient. The required amount will be paid to ICA. Form of payment is by NETS or Cashier's Order
· The security deposit will be refunded to patient after it has been confirmed that both mother and child have left Singapore and there is no breach of any of the conditions stated in the security bond.
SUBMISSION OF APPLICATION FOR PERMISSION TO DELIVER CHILD IN SINGAPORE
1. Checklist:
· Duly completed Forms 14 and V391 (for non-Malaysian), signed by applicant and sponsor. One recent passport – sized color photograph to be pasted on the top corner of Form 14. · Applicant’s travel document with at least 6 months validity · A copy of the travel document personal particulars page and applicant’s Disembarkation / Embarkation (D/E) card · Completed Form V107 · Applicant’s marriage certificate (original and authenticated copy) · Sponsor’s NRIC · Obstetrician’s letter stating applicant’s expected date of delivery and whether there is complication to the pregnancy · Letter from the Embassy or High Commission of applicant’s country stating that the newborn child will follow the parent’s nationality and a valid travel document of that country will be issued to the newborn child.

3. When application is approved (notification is via post):
· Letter of approval will be issued by ICA · A security deposit of between S$1,000 to S$ 3,000 may be required. The payment should be in the form of NETS or Cashier’s Order. · A duly completed Direct Credit Authorisation Form is required to be submitted for refunding purpose.
At time of admission, the full amount counselled for delivery charge will be collected by cash or credit card.
Important note: We wish to highlight that in the event that there develops a high risk complication during her delivery and/or her baby, the estimated bill size to the treatment, length of stay, for both she and/or baby in Intensive Care Unit, may go up from S$ 20,000 to approximately S$ 100,000 and more.
Also, depending on baby's medical condition, the neonatologist may admit the baby. In such case, baby's admission will be separate and baby's charges are based on Class 'A' payment.

ICA Process and Obstetrics Delivery
Kindly see below for the immigration conditions that you would need to fulfill in order to deliver in Singapore as well as the delivery charges in KKH.
Immigration Process to deliver your baby in Singapore
1. Condition
· Applicants must not be more than 6 months pregnant as the application process takes about 4 weeks and applicants may not be allowed to enter Singapore if the pregnancy is more than 28 weeks.
· Applicant is required to be under the care of a KKH Obstetrician and a letter from the Obstetrician is required to support the application. The letter should state the expected date of delivery and whether there is complication to the pregnancy.
2. Local Sponsor
· A local sponsor is required. He or she must be either a Singapore citizen or Singapore Permanent Resident.
